Samsung HeartWise is an application for patients participating in a home-based heart wellness program recommended by your doctor. Samsung HeartWise is designed to be used in conjunction with Samsung watches to promote a healthy lifestyle and is only available to patients participating in the home-based heart wellness program.



Samsung HeartWise tracks your heart wellness data right on your smartphone and automatically sends it to your clinical team. It makes your heart wellness program easy and convenient.
Patients need a referral from a healthcare provider to participate in the program. The application is not accessible to those without a referral.
